Joshua B. Kretchmar
DO, PhD, Physician and Medical Director
Dr. Kretchmar’s interest and belief in the whole patient extends beyond the boundaries of Rejuvenation Medical Primary Care . He keeps active and trained in both Family and Emergency medicine and volunteers in underserved/rural areas and disasters worldwide. He believes all people, everywhere, have a right to medical care.
Dr. Gerald Lee
MD
Dr. Gerald Lee is a primary care physician with over two decades of experience. His career began with seven impactful years in the fast-paced environment of Harborview Emergency Room, providing him with a strong foundation in acute care. For the past 21 years, Dr. Lee has dedicated himself to primary care, emphasizing preventative medicine and wellness to keep his patients healthy and out of the hospital.
Garrett Soames
DMSc, PA-C
Dr. Garrett Soames is an experienced physician assistant with 17 years of practice spanning multiple specialties, with a particular emphasis on emergency medicine, geriatrics, and rehabilitation. Throughout his career, he has provided critical care in both high-acuity Level 1 trauma centers and rural emergency departments, specializing in the complex needs of elderly patients.
With a strong foundation in sports medicine and rehabilitation, Dr. Soames applies his expertise to improve mobility, strength, and overall quality of life for aging populations. His work in skilled nursing and rehabilitation centers has equipped him to manage a wide range of geriatric conditions, including stroke recovery, chronic disease management, and fall prevention.
Dedicated to advancing healthcare access and communication, Dr. Soames also co-founded a medical communication software designed to enhance patient care and support clinical teams.

Meridith is an experienced clinician specializing in Post Acute/Skilled Nursing Home care. With over 10 years of experience, Meridith has developed a deep understanding of the unique medical needs of patients in these settings. Specializing in acute and subacute patients in the skilled nursing home setting, as well as Chronic Disease Management, and providing compassionate support to patients in rehabilitation and long-term care settings, Gibril embodies dedication, his extensive background underscores his commitment to delivering high-quality healthcare tailored to the unique needs of each individual under his care.
Mario Mata
ARNP, Clinician
Mario is a highly specialized Geriatric Advanced Registered Nurse Practitioner (ARNP) with expertise in both clinical and post-acute care settings. He possesses a deep sense of compassion for his patients and works seamlessly with his colleagues as a team player. In addition to his patient care responsibilities, he also dedicates his time to developing and enhancing scheduling and process improvement initiatives to ensure efficient healthcare delivery.
Jessica Cassady
ARNP, Clinician
Jessica Cassady started her medical training in the United States Navy as a Hospital Corpsman assigned to the Marine Corps. After completing her active-duty service, she worked as a Registered Medical Assistant while obtaining her Bachelors of Science in Nursing from Northern Illinois University. She obtained bedside nursing experience in cardiac progressive and intensive care while earning her Masters of Science in Nursing as a Family Nurse Practitioner from University of Illinois at Chicago in 2013. Since then, she has worked in multiple roles including mobile and ambulatory urgent care and family practice, correctional medicine, occupational medicine, neurosurgery/headache management, inpatient behavioral health, and skilled nursing.
Judith Abell

Recently moved from Arizona in order to enjoy cool weather! Originally from the Midwest, but seemed to have continually moved West. Have been a Nurse Practitioner since 1998, providing Primary Care and Wound Care to patients in their homes. Provided care to Veterans while working in a VA Outpatient Clinic in Indiana. A true honor! Looking forward to living in Washington and providing care to patients at Rejuvenation Medical.

Ma. Leah Belo, RN, is the Nursing Home Facility Administrator at Rejuvenation Medical. With 7 years of bedside nursing experience in both Saudi Arabia and the Philippines. Leah coordinates care and operations across our partner nursing facilities and serves as the primary point of contact for scheduling and collaboration with our Chief Medical Officer, Dr. Joshua Kretchmar. With her strong clinical background and attention to detail, she helps ensure seamless care coordination, and a positive experience for both patients and healthcare partners.

Dr. Kretchmar is great! He takes the time to listen and thoroughly understand your situation. He also does a great job of helping you understand all of the options available to you (from traditional medicinal options to alternative healing methods). I recommend him any time!
I have seen a lot of doctors over the years and Dr. Kretchmar is by far my favorite!! He cuts the crap and tells you like it is. He is so friendly and truly cares about your wellbeing. He isn’t afraid to ask the side effects questions which makes me feel less uncomfortable about them.
I also appreciate that he doesn’t push drugs like a lot of doctors do now. He give you options and allows you to decide which of the paths will be right for you. Plus as a bonus, I now don’t dread going to the doctor!! He makes each visit welcoming and inviting.
